Patients researching cosmetic surgery spend weeks on Google before booking a single consultation.

A board-certified plastic surgeon in Miami, FL asked me why a newer practice across town, run by a surgeon with half her years of experience, kept outranking her for "rhinoplasty Miami." Her before-and-after results were better. Her reviews, when you actually read them, were more detailed and more glowing. But her GBP listed her primary category as "Doctor," her photo gallery had 4 images total, and she'd never claimed her RealSelf profile. The newer practice had done all three.
Cosmetic surgery patients are among the most researched healthcare consumers in any specialty. Before a patient books a rhinoplasty consultation or schedules a mommy makeover, they spend weeks or months reading reviews, viewing before-and-after galleries, and researching surgeons online, and increasingly, asking AI tools like ChatGPT or Perplexity directly who the best plastic surgeon in their city is. Every step of that research journey now runs through structured, verifiable information about your practice.
Plastic surgeons who rank on Google Maps with strong reviews, compelling before-and-after photo galleries, and procedure-specific content consistently outperform competitors in consultation volume, regardless of practice size or marketing budget. This guide covers the complete local SEO strategy for plastic surgery and cosmetic surgery practices.
The same approach carries over to neighboring categories, as our guide to local SEO for cosmetic dentists shows.
Primary category: "Plastic Surgeon," the most specific and high-authority category. Not "Doctor," not "Medical Clinic."
Secondary categories:
Business description: Lead with procedure specialties and board certification, stated explicitly, not implied: "[Name, MD] is a board-certified plastic surgeon in [City] specializing in rhinoplasty, breast augmentation, mommy makeover, body contouring, and facial rejuvenation. Our practice offers both surgical and non-surgical cosmetic procedures in a state-of-the-art accredited facility. Consultations available."
Photos: Before-and-after photos are everything in cosmetic surgery SEO. Add 25+ procedure photos with patient consent, organized by procedure type. High-quality clinical photography is non-negotiable, poor quality photos undermine trust more than having no photos at all.
Services: Add every procedure individually:
⚠️ Common Mistake: Listing "Doctor" or "Medical Clinic" as the primary category instead of "Plastic Surgeon." Google uses this field to decide which searches you're eligible for, and a vague category quietly locks you out of the exact procedure searches you want most.
Businesses in adjacent categories face many of the same ranking challenges, which we cover in local SEO for endodontists and oral surgeons.
I call the approach I use with cosmetic surgery clients the Flento Verified-Specialty Method: every piece of content, your GBP description, your procedure pages, your About page, states your specialty, your location, and your board certification explicitly and in plain language, never implied or buried in a bio paragraph. This matters for two audiences at once. Google's local ranking systems weigh explicit relevance signals heavily, and AI Overviews and tools like ChatGPT now surface plastic surgeon recommendations based on structured, verifiable statements they can extract and trust, not vague marketing copy.
How to apply it: on every major page, state plainly: "[Name] is a board-certified plastic surgeon in [City] specializing in [specific procedures]." Repeat this pattern with the exact procedure name on each procedure-specific page, not just once on the homepage.
📊 Flento Data: AI Overviews now appear for a substantial share of local business queries, including cosmetic surgery searches, and the practices most likely to be cited are the ones stating their specialty and credentials in clear, extractable language rather than narrative bios.
Cosmetic surgery keyword research is procedure-driven and research-intensive, which means most search volume comes from patients still deciding, not just patients ready to book.
High-value procedure keywords:
Research and qualification keywords:
The before-and-after gallery is the most important page on a plastic surgeon's website from both an SEO and conversion standpoint, it's usually the first thing a hesitant prospect looks for.
Gallery best practices:
Gallery SEO: Name your image files descriptively (for example, rhinoplasty-before-after-[city].jpg), and add alt text to every image with the procedure name and city.
Every major procedure you perform needs its own page. A single "procedures" page listing everything cannot rank for rhinoplasty, breast augmentation, and mommy makeover simultaneously, each search deserves its own destination.
Each procedure page must include:
Action Step: Audit your website right now for how many of your top 5 procedures have a genuinely dedicated page versus a bullet point on a general list. That gap is your next build.
Cosmetic surgery reviews are transformative stories. A patient describing how their rhinoplasty gave them the confidence they'd been missing for 20 years converts hesitant prospects instantly, more effectively than any clinical description of technique.
When to ask: At the 6-week or 3-month follow-up, when results are visible and the patient is expressing satisfaction, not immediately post-op while they're still healing.
What to say: "Your results have been stunning and you should be so proud. Would you be willing to share your experience with a review? Many patients are exactly where you were a year ago, your story could give them the confidence to take the first step."
📊 Flento Data: Plastic surgery practices with 40+ Google reviews book 3.1x more consultations from Google Maps traffic than those with fewer than 15 reviews.
🔥 Quick Win: Use Flento to send automated post-procedure review requests timed to 6-week and 3-month follow-up appointments, so no satisfied patient's window closes before anyone asks.

Cosmetic surgery directories:
RealSelf is non-negotiable for plastic surgeons. Patients researching cosmetic procedures use RealSelf extensively before they ever contact a practice. An active RealSelf profile with before-and-after photos, answered community questions, and strong reviews can drive a meaningful stream of consultation bookings on its own.
Local press and media. Local news outlets regularly look for expert commentary on health and cosmetic topics. Offering yourself as a source for a local reporter's story on a trending procedure or a seasonal beauty topic earns a genuinely authoritative backlink, something few competing practices bother to pursue.
Blog content ideas:
💡 Pro Tip: Write each answer in direct, quotable sentences near the top of the page. This is the same structure that helps you win Featured Snippets and increasingly what AI tools pull from when generating a recommendation.
The Miami surgeon I opened with fixed her category, uploaded 30 new gallery photos organized by procedure, and claimed her RealSelf profile within a month. Her Maps position for "rhinoplasty Miami" moved from off the first page into the top 5 within the following quarter.

What Google Business Profile category should a plastic surgeon use? "Plastic Surgeon" is the correct primary category, the most specific and highest-authority option available. Generic categories like "Doctor" or "Medical Clinic" dilute your relevance for the exact procedure searches you want to win.
How many Google reviews does a plastic surgery practice need to rank well? Plastic surgery practices with 40 or more Google reviews book roughly 3.1 times more consultations from Google Maps traffic than those with fewer than 15. Consistency and recency matter as much as the total count.
Is RealSelf important for plastic surgeon SEO? Yes, it's one of the highest-impact platforms available. RealSelf is the largest cosmetic surgery review platform in the US, and patients researching procedures use it extensively before contacting a practice. An active, complete profile functions as both a citation and an independent consultation source.
Do AI tools like ChatGPT affect how patients find plastic surgeons? Increasingly, yes. AI Overviews now appear for a significant share of local business searches, and patients are starting to ask AI tools directly for surgeon recommendations. Practices that state their specialty, location, and board certification in clear, structured language across their website and GBP are more likely to be surfaced by these systems.
How long does local SEO take to work for a plastic surgery practice? Expect meaningful movement in 3 to 6 months after fixing GBP categories, building procedure-specific pages, and growing review velocity. Highly competitive markets like Los Angeles, New York, or Miami generally take longer than smaller metro areas.
What's the biggest local SEO mistake plastic surgery practices make? Treating procedures as bullet points on a single generic page instead of building a dedicated, fully optimized page for each one. A patient searching "mommy makeover [city]" is unlikely to land on a page and immediately understand it applies to them if it's buried in a list alongside a dozen unrelated procedures.
Patients planning their procedures are researching right now, comparing surgeons across Google, RealSelf, and increasingly AI tools before they ever pick up the phone. The surgeons who rank on Google Maps, have compelling before-and-after galleries, and build trust through transparent, explicit content are winning the consultations.
